Mental Health Services for Older People in South West Herts are looking for a Social Worker to join our team.

The post holder will work as part of the community team with older people that has a functional mental health condition. They will work alongside Nurses, Occupational Therapists, Psychologists and Psychiatrists; and closely with Community Care Officers to provide an integrated approach that fully involves the person and their family in their health and support.

The team is comprised of four functions - community, crisis, therapies and an Early Memory Diagnosis Service. The four function work together around the person who requires the service without the need for paper referrals. We work with people in Watford and Hertsmere boroughs, and in the district of Three Rivers.

As a team we hold regular reflective sessions and professional peer supervision sessions, along with the normal supervision and appraisals. We are keen to continuously improve, and a recent quality improvement project has seen us change the way our multidisciplinary meeting work, so that it is more productive for every attendee. We have also incorporated dedicated time for multidisciplinary complex case discussions.

Candidates must hold a current registration with The Health and Care Professions Council, and should have experience of post qualification work experience, preferably working with older people.
