It’s an exiting time to join Mental Health Servces for Older People as we move to closer working with Primary Care Networks to be part of local integrated teams that wrap care around people.
Mental Health Services for Older People in South West Hertfordshire is looking to recruit a Community Mental Health Nurse to join our secondary care service, but to work within a Primary Care Network.
As part of MHSOP you will be within the Community Team, and will have close links with our Crisis Function, Care Home Function, Early Memory Diagnosis and Support Service and Therapies - along with all the disciplines that work across and within them. You will however, develop working relationships and links with GPs and other professionals and organisations that work within your PCN so that the people you see get the right care at the right time.
As a team we regularly hold teaching and reflective sessions, and look to embed an open culture of learning through our governance. You will have regular supervision, and an appraisal that looks to support your development.

You must have appropriate UK professional registration.
