The Intensive Support Team provides a rapid, time limited, community based assessment and treatment service for people with learning disability living in Hertfordshire, who challenge services and have additional mental health needs. IST works with service users in their own homes and our aim is to help prevent an unnecessary hospital admission by exploring all other alternatives with the service user and their carers / families.
We are looking to appoint an enthusiastic and forward thinking Learning Disability Nurse to join the Intensive Support Team (IST) in West Hertfordshire. We have two full time, 37.5 hours a week, permanent position, and the post holders will work over 7 day rota.
Applicants will have at least 2 years post qualification experience as a Learning Disability nurse, at Band 5/6 in a community based health service for people with learning disability or inpatient assessment & treatment setting. They must have an awareness of the health needs of this client group, have excellent communication and leadership skills, be keen to extend their skills in a supportive environment, and be able to work effectively in a multidisciplinary team. Applicants should have proven skills in assessment, planning, implementation and evaluation of care for people with learning disability and mental health needs. You will need to be competent with prioritizing service user’s needs and working in a variety of settings. You must have knowledge, understanding and experience of risk assessment, the Mental Health Act, DOLS and The Transforming Care agenda.
The successful candidate will be responsible for a clinical caseload, under the supervision of the Clinical Team Manager and will share responsibility for the support and supervision of junior staff in the team.
You will be passionate about learning disability nursing and be willing to actively participate in the continuing development of the existing nurse team as well as your own professional development (CPD) .
