Main duties of the job

 As a Band 6 Care Co-ordinator, you will be an active member of a skilled multi-disciplinary team.  You will be working together with your colleagues to provide our PATH service in our Watford Team.  Each Care Co-ordinator covers a designated locality – not the whole county.  This means you’ll travel less and work out in the community, both in people’s homes and our local hub buildings. You’ll be passionate in the delivery of recovery based and collaborative approaches to care planning, working flexibly and creatively with people in order to achieve their individual goals. 

Our ‘Caseload Mini Team’ workforce model is unique and highly supportive of collaborative working.   You will work as the Leader of the Mini Team, working closely with a Band 4 & 5 colleague to manage the caseload together.  This role is a real opportunity to build and develop your leadership and delegation skills, as well as maximising the opportunities and support provided by working closely with colleagues, as part of a mini team.

Detailed job description and main responsibilities

Job Summary:
The post holder will work as a member of the multi-disciplinary Community Team, will be responsible for the clinical and operational day to day service delivery, providing a high quality social care service to adults of all ages having First Episode Psychosis. The core functions are to provide assessment, care planning and care co- ordination, including care programme approach, within a framework of recovery which embraces each individual’s right to choice and control.

You may be responsible for supervision, development and teaching of junior staff and/or students as delegated by the team manager.

Person specification

Education/Training

Essential criteria
  • Education to degree level or Academic qualifications equivalent (Diploma/Degree in in Mental Health Nursing /Social Work / Occupational Therapy)
  • Registration with NMC/ HCPC
Desirable criteria
  • Advance Training in mental health
  • Approved Mental Health Professional/Best Interest Assessment training

Experience and Knowledge

Essential criteria
  • At least 2 years of postregistration experience with at least one year at Band 5 (marginally less experience will be considered if applicant can demonstrate competence).
  • Experience of working in acute mental health inpatient/day care services
  • Experience of working in situations of crisis and emotionally demanding environments
  • Experience and knowledge of safeguarding process; dealing with family breakdown; serious mental ill health and risk of selfharm.
  • Experience of partnership working with service users and carers, demonstrating dignity and respect
Desirable criteria
  • Experience of teaching and training

Communication skills

Essential criteria
  • Excellent ability to communicate with a wide range of stakeholders including service users and carers, other professional colleagues, members of other organisations and local communities, verbally and in writing
  • Experience of using electronic recording systems and ability to produce accurate, appropriate and timely reports.

Physical Skills

Essential criteria
  • Car driver essential, or access to a car (unless you have a disability as defined by the Equality Act 2010 which prevents you from driving).
