The East of England Provider Collaborative (EoE PC) is formed of six mental health trusts; Hertfordshire Partnership Foundation Trust (lead provider), Cambridge and Peterborough Foundation Trust (CPFT), Norfolk and Suffolk Foundation Trust (NSFT), North East London Foundation Trust (NELFT), Essex Partnership University Trust (EPUT), parts of East London Foundation Trust (ELFT) and the parts of Central North West London (CNWL). EoE PC has a delegated budget from NHSE through which it funds all CAMHS inpatient provision for the population of East of England. HPFT are the lead provider in the collaborative, and are the employer of this post-holder.
Main duties of the job
We are looking for enthusiastic, highly motivated Registered professionals with a desire to optimize patient flow and discharge pathways across the region, working closely with multi-disciplinary colleagues.
The successful candidate must display a considerable degree of initiative, possess mature judgement. The successful candidate will additionally be required to provide comprehensive, professional, operational, effective leadership and managerial support to within the Single Point of Access and Patient Flow Service; whilst being supported by the Service Manager.
You will also work closely with the Service Manager and will be deputising for them at times ensuring a seamless service is delivered to patients across specialties, ensuring optimum bed capacity and that resources are deployed to meet the challenges faced with patient flow.

Detailed job description and main responsibilities
- The post holder will be responsible for the reviewing and processing of day to day co-ordination of requests for CAMHS, Adult Eating Disorders and Secure inpatient admissions. The Patient Flow Bed Management Team will be responsible for managing bed resources across the East of England (EoE) provider collaborative
- Provide clinical advice to all referrers from across the EoE collaborative regarding admission processes
- To receive referrals from EoE clinicians requesting hospital admissions, to negotiate appropriate placement taking into account service user presentation organisational policy and organisation resources. To take clinical responsibility for bed allocation decisions
- To liaise with services across the collaborative to facilitate organisation of EoE resources to ensure the availability of beds for admissions. Thi
- will include supporting discussions and decisions regarding transfers between wards and to community services to facilitate new admissions as required
- To work proactively with EoE services, social care services and other providers to support discharge, transition and contingency plans to ensure bed availability across services in scope
- Jointly with the bed management service and outreach teams, where clinically indicated, to develop repatriation plans for YP currently placed out of EoE area
- Demonstrate clinical excellence in contributing to the management of complex cases and to assist inpatient MDTs as appropriate in the management and treatment of delayed discharges and Out of Area Treatment processes (OATs)
- To support the bed management/outreach team manager with ensure robust and timely clinical audit systems and the production of associated reports
- To provide clinical supervision to junior staff within the team
- The post holder will regularly undertake QI and development activity as part of the role
- Professional qualifications RN with mental health experience or DipsSW or CQSW or OT.
- Relevant degree or equivalent post qualification training
- Management Qualification or equivalent management experience to masters level
- Evidence of professional registration
- Approved Mental Health Professional
- 3 years senior level experience at band 7
Skills and Knowledge

- Knowledge of legislation and practice relevant to the care group.
- Ability to assess needs of service users and their carers and to implement care planning within the Care Programme Approach framework.
- Ability to work collaboratively with other agencies and as a member of the multidisciplinary team in order to influence practice in relation to early discharge and bed occupancy.
- Ability to use the Trust computerised record system, i.e. Care Notes.
- To have an understanding of mental health community based services
- Ability to consider more than one solution to a potential problem.
- Innovative thinker with the ability to cut through barriers to change.
- Positive and outcome focused approach.
- Ability to take decisions when on call at night and weekends
- Excellent communications skills, oral and written.
- Have knowledge and understanding of Health and Social Care budgets and pressures
- Manage a budget up to £5000.00
- Experience of working with people with mental health problems and working in a care environment.
- Experience of working in a mental health setting as a ward manager or equivalent experience in the community relating to the discharge from hospital of service users
- Knowledge of the National Service Framework and understanding of clinical governance
- Experience in extra contractual referrals (ECRs) and Individual Patient Agreements (IPAs)
- Previous experience training /practising as an ASW/AMHP
- Experience of using a variety of intervention methods
