The Adult Mental Health Rehabilitation Service at HPFT has secured funding to launch  the community based Enhanced Rehabilitation Outreach Service in Hertfordshire, we are looking for an Occupational Therapist who  recognises the importance of instilling hope in our service users and the team they lead, who “gets” recovery and what it means for individuals with Severe mental illness.

The team comprise of Nurses, Occupational Therapists, Psychiatrist, Psychologists and Recovery Support staff.   We are looking for someone who is passionate about empowering people to live independently, lead meaningful lives, engage with their local community and who maintains hope and optimism for every individual using the service. 

You will be part of a highly skilled multi-disciplinary team, you will have leadership experience and have a strong knowledge and skill set of working with a service user group living with severe and enduring mental health issues and will instill a team ethos of using a recovery focused pathway to engage and empower those using the service to work towards achieving their personal goals.  

The Enhanced Rehabilitation Outreach Service (EROS) will use evidence based interventions to support people who present a higher risk of relapse or deterioration when transitioning at any point within the Rehabilitation Care Pathway, so the team will have strong links with the Community Placement Service, Community Housing and Supported Living providers  and the in-patient Rehabilitation service, as well as liaising closely with the person’s community care co-ordinator to ensure that all transfers enable people using our services to experience smooth transitions in their care, without unnecessary delays. 

The EROS team will therefore enable people to move into the most independent placements possible, by providing intensive support at the points of transition.  The rehabilitation service will be implementing outcome measures across the rehabilitation pathway to evidence and celebrate service

You must have appropriate UK professional registration.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
